As a patient where can I utilize your services?
The Wound Care/Prevention Team serves patients in all settings including Acute Care (hospital patients), Long Term Care, Outpatient Rehabilitation, as well as Home Health and Hospice.
How Does the Program Work?
At Memorial Health Center wounds meeting specific criteria are evaluated by the Wound Care/Prevention team with evaluation findings and recommendations communicated to the primary care physician. Recommendations for treatment however, will only be carried out if ordered by the primary care physician.
The primary care physician and Wound Care/Prevention team members are notified of any significant changes in the wound and/or a change in the patient's status. The Wound Care/Prevention Team will re-evaluate the wound on a weekly basis through wound rounds. Our team will evaluate the patient in a holistic manner, assessing all aspects of care and prevention to best serve the patient.
